For patient's
with dental insurance:
Please bring the following
information with you to your appointment ;
Policy number, certificate
number, the name of your insurance company and any information you
have regarding what is covered on your plan... yearly maximums,
frequency allowed for check ups and cleanings etc.
Your insurance company will
give you this information over the phone.
We are happy to submit
claims to your insurance on your behalf and accept payment for any
eligible benefits you are entitled to. However, you are ultimately
responsible for your account, and the cost of your treatment. Any
portion of the cost not covered by insurance is payable at the
appointment.
Please note that your
treatment plan is based on your
needs and your needs may
be different to what is covered by your plan. We always recommend
that you not
base your decision to go
ahead with your treatment on what your plan
covers.
You can avoid expensive
trips to the dentist by brushing and flossing daily and going for
regular cleanings and check-ups. Regular appointments cost money
but they are less expensive than fixing problems that stem from
neglect. |
